New leadership: This Skilled Nursing Facility (SNF) is owned and operated by Stephenson County, but now managed by Transitional Care Management. And we are expanding our team.
With 148 beds, we serve a diverse population of short-term rehab patients, long term care residents and memory care. We have:
- An active therapy dept. achieving terrific functional gains and a remarkable restorative program
- We have a stable team, with a DON that has been with the center for 20 years

Job Description
About you:
The Nursing Home Administrator (LNHA) sets the pace for the skilled nursing facility. You lead by example and you know first impressions matter. People remember you, respect you and want to work with you. You make an impact in your patent’s lives and you strive to make a difference.
- Your number one goal will be to increase clinical programming and marketing initiatives to focus on rebuilding census.
- This will involve interfacing with our union and representing the center at county board meetings.
Qualifications
What we will need from you:
- 3-5 years of experience as an LNHA in a skilled nursing facility
- Ability to build a team and potentially bring some with you
- A love for what you do, and your education and credentialing reflects that
